FCC Form 499 Data Request

We assist companies in completing quarterly and annual FCC Form 499 Data Requests (for each operating year). This form is submitted to the Universal Service Administrative Company (USAC) as a basis for contributions to the USAC organization. USAC distributes supporting funds to the NECA High Cost program, Local Switching Support program, Schools and Libraries Support program, and the Rural Health Care Support program. The USAC Form 499 data request incorporates revenue information, based upon billing records and Part # 32 accounting data, associated with the company's operations. After the USAC Form 499 data request is filed, USAC prepares and submits to the client monthly invoices for the respective company USAC contribution.
 
FCC Form 502 - Numbering Resource Utilization / Forecast (NRUF) Report

ECG assists companies in completing the FCC Form 502. This form must be prepared semi-annually on or before February 1st and August 1st. Form 502 actually has three required forms: company information form, utilization form, and a forecast form. Companies must report utilization and forecast data for all numbering resources. FCC Form 502 is mandatory and must be completed by any company that receives NXX codes from the North American Numbering Administrator (NANPA). If a company requests a code from NANPA, a NRUF from the current reporting cycle must be on file with NANPA in order to receive numbering resources.
